To make a referral to any of the Generations locations, please fax a completed referral form to 612.379.8235 Attention: Intake, or email here. For general questions regarding making a referral please call 612.676.1604. Generations currently accepts: Medical Assistance, County Funding, Private Pay, and most Private Insurance clients. Medical Assistance for all Home-Based services, psychological evaluations and diagnostic assessments, including straight MA and all PMAP's such as; Health Partners, Blue Cross Blue Shield, UCARE, MHP, and UBH (Medica). Private insurance plans for In-Office Therapy, psychological evaluations and diagnostic assessments: UBH (Medica), Blue Cross Blue Shield, Preffered One, Patient's Choice, and Health Partners.
